IMPORTANT: We need your SIGNATURE before we can make any requested changes to your accounts, such as Change of Address, Change of Email Address, Name Change, etc. If you request those changes by email, we will need to contact you to mail us your signed request or to stop in to sign a change form. To protect your accounts, we have to identify you first.

Do not include any personal sensitive information in your message, such as social security numbers, account numbers, etc. We already have this information in your records. If we need more information, we will call you.
